Inspect & Adapt (I&A): Continuous Improvement within SAFe
Inspect & Adapt (I&A) is a crucial part of the Scaled Agile Framework (SAFe) and takes place at the end of each Program Increment (PI). It is the moment when teams and stakeholders together look back at the past PI, celebrate successes, and identify improvements.
I&A helps organizations continuously learn and adapt to changes, both at the team and program levels. This makes it a powerful mechanism to keep not only teams, but the entire organization agile.
Why is Inspect & Adapt important?

How does Inspect & Adapt work?
The I&A event consists of three main phases:
- PI System Demo – Teams collectively demonstrate the results of the past PI.
- Quantitative and Qualitative Measurements – Data is analyzed to gain insights.
- Problem Analysis and Improvement Actions – Teams determine which improvements are needed and create an action plan.
1. PI System Demo
- All teams within the Agile Release Train (ART) collectively present their working product to stakeholders.
- The focus is on the value delivered and not on individual team performance.
- Stakeholders provide direct feedback, which helps prioritize future work items.
2. Quantitative and Qualitative Measurements
In this phase, key metrics are evaluated, such as:
- Program predictability measure – How well did teams meet their planned PI objectives?
- Cycle time en lead time – How efficiently are processes running?
- Quality indicators – For example, number of bugs, performance issues, or technical debt.
- Customer feedback – What do users say about the product?
This data helps teams and leadership objectively determine where improvements are needed.
3. Problem Analysis and Improvement Actions
- Teams identify key problems that affect productivity or quality.
- Techniques such as Root Cause Analysis (RCA) are used to uncover deeper causes.
- Improvement actions are determined and recorded in a concrete backlog for the next PI.
The goal is not only to combat symptoms, but to implement fundamental improvements.
What makes an Inspect & Adapt successful?
- True transparency – Problems should not be hidden or downplayed.
- Engaged stakeholders – Not just teams, but also business owners and management must participate.
- Actionable improvements – The output of I&A should not be optional, but lead to concrete actions in the backlog.
Common mistakes in Inspect & Adapt
I&A becomes a standard meeting without real impact
- It should yield in-depth analysis and real improvements, not just a checklist activity.
Stakeholders do not participate
- The entire system must improve, not just individual teams.
No use of data and metrics
- Without objective data, improvement actions remain based on opinions instead of facts.
Too much focus on small improvements, too little on system problems
- The goal is to resolve structural impediments within the organization.
